Testing and Evaluation Services Are Distributed Into Five (5)
Steps:

Part One: The Intake Appointment

• An initial appointment will be scheduled with a testing clinician. During this
  time, the clinician will conduct a "Behavioral Interview" by asking questions
  and gather information about your developmental history, educational history,
  family history, understand the presenting concern, and intentions for testing
  services.

Part Two: Testing Administration

• Testing administration can be span from One (1) to Four (4) appointments
  lasting from One (1) to Eight (8) hours dependent on clinical need.

Part Three: Scoring and Interpretation

• The clinician will gather the information provided during the "Behavioral
  Interview", in addition to, score and interpret the testing measures taken over
  the course of the last several appointments. This step allows the clinician to
  gain an understanding of how all the information fits together to reach a
  specific diagnostic conclusion or profile. This can take up to Two (2) weeks.

Part Four: Report Composition

• The clinician will take all information, data, and interpretations gathered to
  compose a comprehensive report. This report will outline the assessment
  measures taken, assessment scores, information gathered during the
  Behavioral Interview, clinical interpretations, conclusions, and clinical
  recommendations. This can take up to Two (2) weeks.

Part Five: Feedback Appointment

• A feedback appointment will be scheduled once the report has reached a
  stage of completion. During this time, the clinician will go over the results,
  conclusions, and recommendations to the client (and/or the applicable
  parent/guardian)

How much does testing generally cost?

* Payment executed at the conclusion of each step

• The cost of the entire evaluation process can vary depending on whether or
  not you are using insurance, and what your insurance plan will cover. if you
  are using insurance, we would recommend that you first contact your insurance
  company to determine your benefits. If you are liable for a deductible, we
  would recommend identifying the "insurance rate" for the following "
  procedure codes": 96130, 96131, 96136, and 96137.

• If you are not using insurance, the following services fees are itemized as
  follows: Intake Appointment/Behavioral Interview: $175 First Hour of Testing
  Administration: $170 For Each Additional Testing Hour: $150 Report Scoring,
  Interpretation, Composition: $150/Hour Feedback Appointment: $175
